Job summary

Qualifications

• Minimum 5 years’ experience in Social Services with 1-year in Case Management.
• Experience with high needs, low-income families and housing instability.
• Knowledge of community resources and social service systems.

Responsibilities

• Provide case management and service coordination for clients in PSH.
• Work closely with community agencies and coordinate benefits.
• Facilitate group activities and programming for clients.
