MHA Inc. in Chicopee, MA is seeking a dedicated Case Manager for the State Opioid Response Rapid Rehousing Program.

You will perform outreach, assess needs, connect participants with housing options, and guide them toward independent living with trauma-informed support. The role emphasizes collaboration with landlords, benefits navigation, budgeting, and arranging move-in logistics, while maintaining accurate case files and documentation.

Qualifications

• Must have a valid driver’s license with acceptable driving history.
• Pass CPR and first aid certifications.
• Experience supporting individuals with mental health or substance use challenges preferred.

Responsibilities

• Assess eligibility and enroll participants in the Rapid Rehousing program.
• Conduct community outreach to identify needs and recovery goals.
• Assist in locating affordable housing and coordinate viewings.
• Provide tenancy support and landlord communication guidance.
• Coordinate move‑in logistics and utilities setup.
• Support budgeting, benefits navigation, and transportation to appointments.
• Maintain participant files and program documentation.
