Victory Programs Incorporated is seeking a Housing Stabilization Case Manager in Boston to coordinate direct case management for residents transitioning from shelter to permanent housing. The role supports 24 months post-exit with a focus on housing options, lease-up, and access to basic needs.

You will collaborate with recovery services, medical providers, and community partners, maintain thorough records, and participate in team supervision and trainings as required.

Formación

• Bachelor degree with 1-2 years’ experience preferred, or High school diploma with 3 or more years of client advocacy experience.
• Experience advocating for clients with government assistance agencies, housing authorities, private landlords or realtors preferred.
• Openness to harm reduction philosophy.
• Ability to work independently and cooperatively as a member of a professional team.
• Strong organizational, written, and verbal communication skills.
• Strong computer skills, including Google office suites and Internet use.
• Demonstrated ability to work successfully in multicultural settings with diverse client groups, including people with mental health concerns and substance use disorder.
• Must be CPR certified (may be obtained upon hire)
• Complete TB screening (may be obtained upon hire)
• Three strong work related professional references; at least 2 from manager or supervisor.

Responsabilidades

• Maintain professional ethics, boundaries and client confidentiality.
• Follow program policies and procedures.
• Implement best practices in alignment with Housing First philosophy.
• Identify housing options; communicate with landlords; view units with clients.
• Assist clients in locating units with Rapid Re-Housing rental assistance.
• Support clients with lease-up, move-in, and access to basics.
• Collaborate with recovery and medical services teams; coordinate referrals.
• Obtain consent to coordinate referrals and housing search.
• Identify supports to maintain housing during rapid re-housing period.
• Plan for housing beyond rapid re-housing including income and benefits.
• Meet with clients where they are; provide weekly case management.
• Accompany clients to appointments and follow up on applications.
• Document individualized support plans; set measurable goals.
• Continue to assess and refer to community-based services.
• Promote community participation and resource access.
• Develop a broad housing resources network in Boston.
• Provide coordination with existing programs to maximize access to services.
• Maintain timely records and required assessments.
• Keep client files accurate and up-to-date.
• Complete agency or funder reports as needed.
• Attend weekly meetings and supervision; pursue training needs.
