Job summary

MHA Inc. is seeking a compassionate Case Manager to support individuals transitioning from homelessness into supportive housing. You will provide trauma-informed, person-centered services, coordinate assessments, and help participants stabilize income, health, and independence.

You will work Monday through Friday, 9am–5pm, with a focus on housing stability, benefits access, and skill development, within a compassionate, team-oriented environment.

Qualifications

• Six months or more of relevant experience required.
• Must be at least 21 years old and able to obtain CPR and First Aid certifications.
• Valid driver's license, vehicle insurance, and reliable transportation.
• Ability to work independently and as part of a team.
• Strong organizational, communication, and interpersonal skills.

Responsibilities

• Coordinate documentation for program entry (homelessness verification, IDs, income verifications, etc).
• Conduct initial assessments, updates, and service plans.
• Collaborate with a multidisciplinary team to develop Individualized Action Plans (IAPs).
• Implement IAPs and document progress.
• Develop, monitor, and revise safety plans with participants and team.
• Assist participants with transition to stable housing.
• Communicate with Chestnut Crossing staff about participant needs.
• Support tenants with housing responsibilities and regulations.
• Guide participants toward employment, vocational training, or education.
• Help participants access public benefits and community resources.
• Encourage community integration and participation in activities.
