Job summary

MHA Inc. in Chicopee, MA is seeking a Case Manager to provide trauma-informed, person-centered support to individuals transitioning from homelessness into studio apartments within a congregate housing site.

The role emphasizes stability, employment goals, and access to medical care, with ongoing coaching and documentation. The position includes open shifts Monday through Friday, 9am–5pm, with some flexibility, and pays $20 per hour.

Qualifications

• Valid driver’s license and reliable transportation.
• Ability to work independently and as part of a team.
• Strong organizational, communication, and interpersonal skills.
• Capacity to adapt to evolving participant needs.
• Must be at least 21 years of age.
• Must be able to obtain CPR and First Aid certifications.
• High school diploma or equivalent.
• Minimum of six months of relevant experience.
• Experience with mental health/substance use challenges or housing instability preferred.

Responsibilities

• Coordinate documentation required for program entry, including homelessness verification, assessments, eligibility screenings, and IDs/birth certificates/Social Security cards/income verifications.
• Complete initial Adult Comprehensive Assessments, updates, and service plans.
• Develop Individualized Action Plans (IAPs) with participant voice, choice, strengths, and needs.
• Implement IAP strategies and document progress.
• Develop, monitor, and revise safety plans with participants and the team.
• Assist participants with the transition to stable housing.
• Maintain strong communication with Chestnut Crossing staff.
• Support tenants in housing responsibilities and regulations.
• Assist participants in seeking employment, vocational training, or education.
• Educate and assist participants in accessing public benefits (SS, EAEDC, SNAP, PVTA, Fuel Assistance).
• Encourage community integration by helping access public spaces, support groups, and activities.
• Promote skills in ADLs, money management, transportation, and social participation.
