Qualifications

• Bachelor's degree required; related field preferred with case management experience.
• Alternative: high school diploma or equivalent plus four years of relevant service experience.

Responsibilities

• Identify and maintain a caseload of unengaged/disengaged clients and build trust.
• Conduct field-based outreach including home visits and community visits.
• Develop relationships with community partners to enable warm hand-offs.
• Perform initial assessments and create outreach/care plans with clients and Case Manager.
• Use motivational strategies to re-engage clients and support housing goals.
• Provide crisis intervention, housing stabilization, and resource linkage.
• Coordinate housing navigation and document housing voucher needs.
• Maintain progress notes, track contacts, and report monthly outcomes.
• Maintain confidentiality per HIPAA and sustain data accuracy in records.
