Job summary

Mental Health Association, Inc. seeks a Case Manager to provide trauma-informed, person-centered support to individuals in recovery with histories of homelessness.

The role focuses on transitioning participants to studio apartments at a congregate supportive housing site, coordinating documentation and service plans, and traveling to the main office in Chicopee. This on-site position involves ongoing support after housing, assisting with benefits, employment, and community integration, and

Qualifications

• Valid driver’s license and reliable transportation.
• Ability to work independently and as part of a team.
• Strong organizational, communication, and interpersonal skills.
• Must be at least 21 years old with CPR and First Aid certifications.
• High school diploma or equivalent required.
• Minimum of six months of relevant experience.

Responsibilities

• Coordinate documentation for program entry, including homelessness verification, assessments, and IDs.
• Complete initial adult assessments, updates, and service plans.
• Collaborate with a multidisciplinary team to develop Individualized Action Plans.
• Implement IAPs and document progress.
• Develop and revise safety plans with participants and the team.
• Assist with transition to stable housing and ongoing tenancy support.
• Maintain communication with Chestnut Crossing staff and navigate housing responsibilities.
• Support participants in seeking employment, training, or education.
• Educate and assist with public benefits (Social Security, EAEDC, SNAP, PVTA, Fuel).
• Encourage community integration and participation in activities.
