Job summary

The MHA (Mental Health Association) is seeking a Case Manager to provide trauma‑informed, person‑centered support to individuals transitioning from homelessness to studio apartments within a congregate housing site.

You will coordinate documentation, conduct assessments, develop action plans, and connect participants with housing, medical care, employment, and public benefits while maintaining thorough records and collaborating with the Chestnut Crossing team.

Qualifications

• Bilingual English/Spanish encouraged.
• Valid driver's license and reliable transportation.
• Must have at least six months of relevant experience.
• High school diploma or equivalent is required.
• CPR/First Aid certifications preferred or obtainable.

Responsibilities

• Coordinate all documentation required for program entry, including homelessness verification, eligibility screenings, IDs, birth certificates, and income verifications.
• Complete initial Adult Comprehensive Assessments, updates, and service plans.
• Collaborate to develop Individualized Action Plans (IAPs) using participant voice, strengths, and needs.
• Implement IAP strategies and document progress as required.
• Develop, monitor, and revise safety plans with participants and the team.
• Assist participants with transition to stable housing.
• Maintain communication with Chestnut Crossing staff.
• Support tenants in housing responsibilities and communications with management.
• Assist participants in seeking employment, vocational training, or education.
• Educate and assist participants in accessing public benefits and resources.
• Encourage community integration through spaces, groups, and activities.
• Promote skills in ADLs, money management, transportation, and social participation.
