Gandara Center in Springfield, MA invites a Supportive Housing Case Worker to provide ongoing case management for individuals and families in recovery from substance use. Services are delivered in offices, homes, and in the community to support housing stability.

The role emphasizes assisting with housing, benefits access, and life skills training, within a diverse, respectful team environment.

Qualifications

• Ability to work with diverse populations.
• Lived experience preferred.
• Valid driver’s license and own vehicle.
• Bilingual / Bicultural encouraged to apply.
• Commitment to racial equity and inclusive practices.
• Knowledge of community resources.
• Client-centered service delivery.

Responsibilities

• Establish positive professional relationships with participants.
• Provide and coordinate housing stability support.
• Provide case management including intakes and assessments.
• Assess and facilitate access to benefits (SSI, SSDI, SNAP, TANF).
• Provide linkage to community resources for economic independence.
• Teach life skills such as budgeting and transportation.
• Provide advocacy and help with self-advocacy for needed services.
• Work with participants to create and monitor an individual service plan.
• Complete all required reports and tracking for participants and funding sources.
• Ability and willingness to drive the agency vehicle.
• Respond to after-hours calls and emergencies.
• Attend required meetings and trainings.
• Respect diverse world views and communication styles.
