Job summary

MHA Inc. is seeking a Case Manager in Chicopee, MA to provide outreach, assessment, and engagement for participants in the BSAS Low-Threshold Housing Program.

You will support individuals in recovery and homelessness to transition into housing, maintain housing, and access essential services with trauma-informed, person-centered care. The role requires collaboration with a multidisciplinary team, documentation, and adherence to agency protocols.

Qualifications

• Must be at least 21 years old.
• Valid driver’s license and reliable vehicle.
• Certifications in CPR and first aid must be maintained.

Responsibilities

• Coordinate and complete program entry documentation: screenings, assessments, treatment plans, notices.
• Collaborate with multidisciplinary team to develop Individualized Action Plans (IAPs).
• Develop, implement, and monitor individualized support strategies and crisis plans.
• Assist participants in securing and maintaining housing, including landlord outreach and housing education.
• Provide tenancy support: moves, utilities setup, address changes, housing standards.
• Support access to employment, education, benefits, and community resources.
• Promote daily living skills and recovery-oriented activities.
• Provide emotional support and harm-reduction counseling, accompany to medical/behavioral health appointments.
