Job summary

Mental Health Association - Western MA seeks a Case Manager to provide trauma-informed, person-centered support for individuals in substance use and/or mental health recovery with a history of homelessness. The role helps participants transition from homelessness to studio apartments within a congregate housing site and continues to provide ongoing support after housing is secured.

The position is on-site at Chestnut Crossing with significant community work and travel to MHA’s Main Office in

Qualifications

• High school diploma or equivalent required.
• Minimum six months of relevant experience.
• Valid driver’s license and reliable transportation.
• Must be able to obtain/maintain CPR and First Aid certifications.
• Ability to work independently and as part of a team.

Responsibilities

• Coordinate all documentation required for program entry, including homelessness verification and IDs.
• Complete initial Adult Comprehensive Assessments, updates, and service plans.
• Collaborate within a multidisciplinary team to develop Individualized Action Plans (IAPs).
• Implement strategies outlined in the IAP and document progress.
• Develop, monitor, and revise safety plans with participants and the team.
• Assist participants with transitioning to stable housing.
• Maintain communication with Chestnut Crossing staff.
• Support tenants in navigating housing responsibilities and regulations.
• Assist participants in seeking employment, training, or education.
• Educate participants on accessing benefits and community resources.
• Encourage community integration and participation in activities.
