Job summary

LA Family Housing is seeking a Housing Stabilizer to provide intensive, home- and community-based case management for households transitioning out of homelessness. You will conduct assessments, plan goals, connect to housing, health, and income resources, and coordinate services in partnership with community providers.

The role requires collaboration across teams, adherence to program standards, and crisis intervention to support clients facing medical, mental health, and substance use barriers

Qualifications

• At least 1 year of work history in related field.

Responsibilities

• Engage and develop rapport with households transitioning out of homelessness using Trauma-Informed Care, Harm Reduction, and Housing First.
• Provide intensive case management including living skills, housing stabilization, money management, and links to health services.
