Job summary

The People Concern is seeking an ICMS Case Manager to support housing stability and community reintegration for Clients experiencing homelessness. You will work with a multidisciplinary team to provide intensive case management, coordinate services, and help Clients gain independent living skills in a trauma-informed, harm-reduction framework.

Qualifications

• High School Diploma or GED or Equivalent.
• Valid CA Driver’s license and reliable vehicle.
• Willing and able to transport Clients in personal vehicle.
• Able to work in a harm reduction model with Clients.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management services (up to 40 if low acuity).
• Develop relationships with Clients with a focus on independence and improved health.
• Collaborate with LA County DMH and DHS programs to enhance coordination within the Clients’ treatment team.
• Engage Clients with an approach emphasizing Client choice, harm reduction, and psychosocial rehabilitation.
• Assist in screening, intake, and enrollment of Clients including orientation to policies and goals.
• Complete comprehensive biopsychosocial assessments to guide treatment planning.
• Write and maintain progress notes and data in electronic databases as required.
• Coordinate appointments, transportation, and follow-up services for Clients.
• Transport Clients in personal vehicle to agencies and providers in the LA area.
• Teach life skills such as budgeting, housekeeping, and appointment scheduling.
