Job summary

Share's team seeks a dedicated Case Manager to support permanent supportive housing and prevention programs, applying the Housing First model to help homeless individuals access and sustain housing. You will provide caseload management, housing navigation, and individualized planning to promote stability.

The role requires a Bachelor's degree and 2–3 years of experience, with emphasis on confidentiality, organization, and collaborative work with housing partners and clients to meet lease

Responsibilities

• Maintain an active caseload and complete eligibility assessments.
• Provide housing navigation and support to secure permanent housing.
• Assist clients with lease requirements and rent payments.
• Collaborate with clients to develop crisis prevention and response plans.
• Complete monthly reporting forms and documentation.
