Job summary

West Virginia Coalition to End Homelessness (WVCEH) seeks a Housing Stabilization Case Manager to support individuals and families from homelessness to permanent housing. This is a community-based role requiring compassionate, organized, and resourceful candidates who can work independently.

The ideal candidate will manage a caseload of 15-20 households, meet participants weekly in various settings, and coordinate housing resources. Travel and sometimes non-traditional hours may be required.

Qualifications

• Bachelor's degree preferred but not required.
• Valid driver's license and reliable transportation.
• Experience with homelessness or social services preferred.
• Willingness to travel throughout the assigned service area.

Responsibilities

• Provide individualized, community-based case management to approximately 15-20 households.
• Meet with participants weekly in homes or community settings, with additional contact as needed.
• Assist participants with identifying and obtaining permanent housing.
• Help participants obtain documentation to establish eligibility for housing programs and resources.
• Build relationships with landlords to identify housing opportunities and support placements.
• Provide ongoing housing stabilization and tenancy support, including budgeting and crisis planning.
• Connect participants with benefits and supportive services (healthcare, employment, education).
• Enter services into HMIS client database in real time, within 48 hours.
• Maintain flexibility to provide services during non-traditional hours when needed.
• Participate in required trainings and professional development.
