Job summary

Mid-Willamette Valley Community Action Agency is seeking a Residential Housing Coordinator to help participants secure and stabilize housing and coordinate services. You will assess eligibility, develop action plans, and work with landlords and local agencies to remove barriers to housing access.

You will document progress, maintain files, and collaborate with a Housing Authority partner while ensuring accurate data entry into HMIS and MOTS systems.

Qualifications

  • High School Diploma or GED required.
  • Two years of Case Management or Social Service experience required.
  • Experience working with Veterans required.
  • Veteran status preferred.
  • Equivalent combination of education and experience may be considered.

Responsibilities

  • Assess participant eligibility and gather documentation as required.
  • Complete participant assessments to determine continued eligibility and needs.
  • Develop and coordinate individualized action plans.
  • Coordinate services with Peer Support Specialist and Program Coordinator for data management and reporting.
  • Monitor progress and maintain detailed case notes.
  • Maintain organized participant files.
  • Assist participants in locating and stabilizing housing.
  • Develop and maintain relationships with local landlords.
  • Identify needs for Barrier Removal Funds (deposits, utilities, IDs, etc.).
  • Coordinate financial assistance with Salem Housing Authority.
  • Network with community agencies to stay informed about services.
  • Provide information, referrals, advocacy, and follow-up as needed.
  • Enter client data into HMIS and MOTS systems.
  • Prepare data and evaluation reports as required.
  • Other duties as assigned.
