Mid-Willamette Valley Community Action Agency is seeking a dedicated Case Manager to assist clients in navigating resources and securing housing. This role emphasizes engaging with unsheltered or vulnerable populations, coordinating assessments, and linking participants to essential services.

The position requires strong planning, organization, and communication skills, with opportunities for growth through hands-on experience and professional development.

Qualifications

• Two years of related social service experience, direct client care, or equivalent combination.
• Ideal candidates will have an advanced degree with two+ years homeless service delivery experience and mental health certifications and/or licensing.
• Basic Life Support/First Aid Certification within 30 days; Food Handler's Card within 30 days.

Responsibilities

• Works directly with clients to ensure access to housing assessments and resources.
• Assess eligibility and aid in gathering documentation for intake, enrollment, and shelter placement.
• Provide direct service including case management and housing placement support to promote self-sufficiency.
• Develop Action Plans identifying needs, barriers, and strengths.
• Establish and maintain relationships with community partners.
• Maintain client-tracking, service delivery, and confidentiality; ensure compliance with funding requirements.
