The Salvation Army is seeking a dedicated Case Manager to provide housing services support and detailed case management for individuals and families. The role involves interviewing clients, documenting needs, and linking to community resources.

Requires a High School diploma plus two years in case work or related fields; strong communication and admin skills, and a valid driver's license. Travel between Davenport and Moline may be needed.

Qualificações

• High School diploma or equivalent and 2+ years in case work or related field.
• College coursework in human services preferred.
• Flexible, able to multitask and prioritize.
• MS Word/Excel proficiency and web-based data systems access.
• Administrative skills: data entry, 10-key, filing.
• Strong written and spoken communication, teamwork.
• Basic math skills and handling confidential information.
• Detail-oriented with problem-solving ability.

Responsabilidades

• Conduct client interviews to assess needs for assistance.
• Complete documentation and maintain client files (intake, service plans).
• Maintain documentation in HMIS, SIMS, and other systems.
• Educate clients on community resources and referrals.
• Provide financial literacy and budget counseling.
• Process paperwork for eligible participants per funding sources.
• Submit funding requests on time as required.
• Advocate for clients with the care team.
• Provide 90-day follow-up after program completion.
• Follow Salvation Army policies and emergency procedures.
• Assist with quarterly reviews and compliance checks.
• Network with community agencies for better outcomes.
• Participate in community and task force meetings.
• Travel between Davenport and Moline offices; driving required.
• All other duties as assigned.
