Job summary

The Salvation Army is seeking a Housing Case Manager to guide clients from housing instability to self-sufficiency. You will provide individualized case management, connect clients with community resources, and build partnerships with local housing providers.

Responsibilities include assessing housing needs, developing case plans, and maintaining records to ensure clients progress through a continuum of care. Applicants should have a social work background and a valid Wisconsin driver’s license.

Qualifications

• Bachelor’s Degree in Social Work or related field is required.
• Two years of case management experience preferred, with mental health/AODA experience beneficial.
• Valid Wisconsin Driver’s License with insurance clearance; CPR/AED/First Aid with Naloxone within 60 days of hire.

Responsibilities

• Interview applicants to assess housing needs and program eligibility.
• Provide ongoing, individualized case management to help clients achieve self-sufficiency.
• Refer clients to community resources and document outcomes in the management system.
• Maintain budgeting guidelines and track financial assistance within allotted amounts.
• Prepare and report monthly statistics and outcomes to the Director of Social Services.
