Job summary

The Salvation Army USA Western Territory is seeking a Housing Program Case Manager to provide comprehensive case management and referrals to individuals experiencing homelessness, helping them access permanent housing through coordinated services.

You will manage 25+ caseloads, conduct assessments, develop housing plans, and collaborate with partner agencies, with field-based duties and a trauma-informed, housing-first approach.

Qualifications

• BA/BS in social work or related field; 1 year experience with special needs or homeless populations.
• High school diploma with 2-3 years direct mental health or intensive case management experience.

Responsibilities

• Provide crisis response focused on housing resources and rapid access to alternatives.
• Offer active listening to identify viable housing options within networks and resources.
• Identify financial assistance and connect participants to housing placement services.
• Ensure compliance with contracts, policies, and privacy laws governing the program.
• Enroll participants and document eligibility; assist with documents for housing placement.
• Develop individualized housing plans with goals and timeframes.
• Manage a caseload of 25+ participants and monitor progress.
• Conduct weekly face-to-face or approved case management visits as needed.
• Complete psychosocial assessments and monthly action plans for housing placement.
• Strengthen job readiness with coaching on resumes, applications, and interviewing.
• Assist in budgeting and income planning; track participant finances.
• Collect financial documents and maintain accurate records.
• Complete VISPDAT/Next Step Tool and coordinate CES matching systems.
• Collaborate with other agencies to provide access to housing resources.
