Job summary

The Salvation Army USA Western Territory is seeking a Case Manager to provide comprehensive assessment, planning, implementation and evaluation of client needs to promote continuity of care and cost effectiveness in housing programs in Los Angeles County.

The role involves coordinating housing readiness, assisting with entitlements, maintaining HMIS records, and collaborating with multiple agency partners to support residents’ well-being and transition to permanent housing.

Qualifications

• Bachelor of Arts in Social Work or Associates Degree or related field preferred.
• Minimum of one-year experience working with low-income and/or homeless individuals and families.
• Must possess a Valid Class C California Driver's License.
• Must pass a criminal background check; Protect the Mission policy for work near children.

Responsibilities

• Provide strength-based case management and service coordination to assist clients in obtaining and maintaining permanent housing.
• Maintain and update client information in HMIS and ensure record accuracy for reports.
• Develop and monitor individualized service plans; conduct re-evaluations as needed.
• Assist clients with access to financial entitlements (CalWORKs, GR CBEST) and health care.
• Coordinate discharge planning and housing readiness activities.
• Network with external programs to maximize participant outcomes and attend weekly meetings.
