Job summary

The Salvation Army USA Western Territory seeks a Case Manager I to deliver holistic, strength-based housing support. You will remove barriers to housing, build individualized housing plans, and help guests save toward sustainable housing.

Responsibilities include coordinating housing documents, linking guests to on-site and off-site resources, and participating in CES housing match meetings while maintaining records in HMIS/WellSky.

Qualifications

• BSW or equivalent degree (preferred).
• Minimum 1-year experience in case management working with people in recovery, mentally ill and/or homeless.
• 1-year experience with HMIS (preferred).
• Experience with tenant/landlord relationships.
• Must be in good physical health and mental health, capable of meeting the demand of the position.

Responsibilities

• Provide holistic, strength based, harm reduction case management in housing program.
• Maintain an active case load and meet with guests four times a month.
• Assist guests with obtaining documents needed for housing.
• Schedule and facilitate weekly life skills groups related to housing needs.
• Enter guest case notes into internal and external databases in a timely manner.
