Job summary

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, Sociology, Public Administration, or a related field preferred.
• Relevant case management or human services experience may be considered in combination with education and other qualifications.
• Experience with homelessness, housing instability, or barriers to stability preferred.
• Knowledge of housing stabilization and NYC social services systems preferred.
• Strong organizational, engagement, advocacy, problem-solving, and documentation skills.

Responsibilities

• Assist participants with neighborhood orientation and connections to community resources.
• Conduct intake and needs assessments; develop individualized service plans.
• Maintain participant contact and monitor progress toward goals.
• Coordinate referrals to public benefits, healthcare, employment, and education.
• Assist with applications, documentation, appointments, and follow-up.
• Advocate for participants and support independent living skills.
• Maintain accurate, confidential case records and participate in supervision.
