Job summary

Homefull is seeking a Case Manager to coordinate all aspects of client care and case plans, helping clients obtain and maintain housing stability and independent living skills. You will work as part of a multi-disciplinary housing-focused team, conduct home visits, and document services in partnership with clients to improve outcomes.

The role requires a Bachelor’s degree in related fields and a working knowledge of homelessness.

Qualifications

• Bachelor's degree in social work, counseling, psychology, or related field.
• Experience/knowledge of homelessness and/or marginalized communities preferred.
• Proficiency with Word, Excel and Outlook.

Responsibilities

• Coordinate client care and support across housing-focused programs.
• Develop and monitor individualized service plans (ISPs) and housing stability goals.
• Maintain accurate clinical documentation and client records.
• Meet productivity targets and participate in ongoing training.
