Job summary

Community Resource Center is seeking a Social Services Case Manager to provide housing-focused, participant-centered case management and supportive services. You will help individuals and families identify barriers to housing stability and work toward personalized goals.

The role involves intake, eligibility, assessments, case planning, and regular follow-up, with travel to meet participants where they are. Strong communication and collaboration with partners are essential.

Qualifications

• Associate degree in Social Work, Psychology, Sociology, Human Services, or a related field required; Bachelor’s degree preferred.
• 1+ years of social services experience, including case management, crisis intervention, counseling, housing services, resource navigation or other direct client services.
• Knowledge of homelessness prevention and intervention strategies and housing programs is desirable.
• Ability to work independently in office and community settings with sound judgment.
• Strong organizational, time-management, and documentation skills.
• Excellent written and verbal communication; professional relationships with participants, landlords, and partners.

Responsibilities

• Provide information, resource navigation, crisis intervention, and eligibility screening to participants and callers.
• Manage a caseload with intake, eligibility, case plans, and progress reviews.
• Engage participants through meetings, in-office, telephonic, virtual, and in-home visits as needed.
• Conduct home visits to support housing stabilization and connect to resources.
• Travel locally to meet participants, inspect housing, meet landlords, and attend off-site meetings.
• Support contract outcomes through housing and supportive services activities, benefits assistance, and compliance requirements.
• Advocate and coordinate services with landlords, healthcare, public benefits, and community agencies.
• Maintain accurate participant records and documentation per agency standards.
• Exercise sound judgment and consult supervisors as needed; participate in supervision and trainings.
• Attend required meetings and perform other duties as assigned.
