Job summary

Making Opportunity Count is seeking a Stabilization Case Manager in Massachusetts to provide ongoing case management for families transitioning from shelter to apartments. The role focuses on continuous barrier reduction and helping families develop skills and resources to sustain housing, with programs typically lasting up to three years.

Qualifications

• Associate's or Bachelor's degree in Human Services or related field.
• Minimum two years of experience in human services.
• Minimum seven years of human service experience.

Responsibilities

• Monitor intake, assessment, and data entry for program clients.
• Regular communication with the Property Owner for program compliance.
• Assist families with tenancy concerns, budgeting, and maintenance reporting.
• Assist families with enrolling in entitlement benefits (SNAP, TAFDC, SSI).
• Work collaboratively with community partners.
• Conduct regularly scheduled meetings with families.
• Keep supervisor aware of concerns or potential problems.
• Attend meetings and trainings as needed.
• Perform all other duties as assigned by Shelter Manager and/or Director of Shelter Case Management.
