Job summary

Making Opportunity Count Inc. is seeking a dedicated Shelter Case Manager to support families in shelter, developing individualized rehousing plans and coordinating with staff to ensure safe, stable, affordable housing outcomes.

You will conduct home visits, document case management efforts, and help clients access essential resources. The role calls for collaboration, strong budgeting skills, and compassionate engagement with diverse populations.

Qualifications

• Associate's degree in human services, or related field.
• Minimum 1 year of case management experience focusing on pre-tenancy and tenancy-sustaining services.
• Experience with budgeting and basic math.
• Ability to collaborate with diverse populations and stakeholders.

Responsibilities

• Provide comprehensive case management to families in shelter.
• Develop individualized rehousing plans with clear goals and action steps.
• Generate and maintain detailed case notes in ETO within 24 hours of contact.
• Coordinate monthly rehousing meetings and document meeting notices.
• Review barriers to housing and help clients access community resources.
• Assist with home visits and community-based support as needed.
