Job summary

Qualifications

• Bachelor’s degree in a related field is preferred.
• Experience with homelessness or housing stability is preferred.
• Knowledge of NYC social services systems is a plus.

Responsibilities

• Assist participants with orientation and connections to resources.
• Conduct intake and needs assessments with assigned participants.
• Develop individualized service plans based on strengths and goals.
• Maintain contact and monitor progress toward service-plan goals.
• Coordinate referrals to public benefits, healthcare, and education.
• Assist with applications, appointments, and follow-up activities.
• Advocate for participants and support self-determination.
• Maintain confidential and accurate case records.
• Participate in supervision, case conferences, and trainings.
