Job summary

Save The Family seeks a Case Manager to support direct services for families in locating and securing rental housing within the community. You will assist with paperwork, referrals, and goal setting, while maintaining open communication with clients and supervisors.

Qualifications

• Bachelor’s degree in social services from an accredited college or university preferred, or five years related field experience.
• Must be able to speak, read, and write English proficiently.
• Strong cultural competence for diverse communities; inclusive of all people.

Responsibilities

• Provide ongoing case management to help families secure permanent housing and resources.
• Meet clients regularly to complete assessments, sign service plans, and monitor progress.
• Develop and monitor service plan goals with the team and families.
