Qualifications

• 5 years of case management in the last 7 years may substitute education.
• Strong verbal and written communication; English required; multilingual a plus.

Responsibilities

• Develop housing and service plans with residents to secure and sustain permanent housing.
• Manage resident behaviors, enforce program rules, promote safety.
• Develop Independent Living Plans tailored to residents’ incomes and assets.
• Provide housing leads, advocate with landlords and brokers.
• Assist with housing interviews, applications, and DHS-related processes.
• Escort residents to interviews and inspections; enroll in care programs.
• Coordinate with education and immigration services; provide referrals.
• Meet residents weekly; update plans; document interactions and outcomes.
