Qualifications

• Experience with chronically homeless adults, severe mental illness, and/or substance abuse.
• Working knowledge of service organizations in Charlotte Mecklenburg area.
• Ability to work independently and collaboratively.

Responsibilities

• Provide case management services for chronically homeless individuals.
• Conduct home visits and provide treatment services.
• Assess medical needs and connect clients to resources.
• Manage participant records and develop client-centered plans.
• Assist participants in obtaining income and achieving self-sufficiency.
• Handle emergencies, conflicts, and maintain a safe environment.
