Job summary

Roof Above in Charlotte, NC seeks a skilled Case Manager to help chronically homeless individuals obtain stable housing through evidence-based services.

You will visit tenants’ homes, coordinate with health and social supports, and work with landlords to ensure timely rent payments.

The role requires a degree in human services (or equivalent experience), a valid driver's license, and strong crisis management and communication skills.

Qualifications

• Bachelor’s or Master’s degree in a human services field or commensurate experience.
• Minimum of three years case management experience with adults who experienced chronic homelessness, etc.
• Experience with harm reduction principles and motivational interviewing.
• Knowledge of Charlotte Mecklenburg service systems.
• Ability to work independently and with clients, service providers and landlords.
• Valid driver’s license and safe driving record.
• Strong verbal and written communication and computer proficiency.

Focusing on individuals experiencing homelessness, Roof Above offers everything from street outreach to shelter to Rapid Rehousing to Permanent Supportive Housing. Roof Above’s services and programs reach over 1,200 individuals every day across multiple campuses in Charlotte. Roof Above was born through the merger of Urban Ministry Center and Men’s Shelter of Charlotte in 2019.PROGRAM OVERVIEWThe Rise on Clanton PSH Program is an 88-unit apartment complex providing permanent supportive housing to men and women who have experienced homelessness. Our philosophy follows a housing first” and “harm reduction” approach.POSITION SUMMARY:Roof Above operates a single-site housing programs that addresses chronic homelessness by providing subsidized housing and support services. The case manager will visit tenants in their homes, working with them to ensure their basic needs are being met while exploring access to appropriate community services such as, mental health counseling, medical care, substance abuse treatment, peer support, employment opportunities, and any other relevant support.ESSENTIAL FUNCTIONS:Provide case management services to help participants successfully transition from chronic homelessness to housing. Manage a caseload of approximately 24 individuals.Conduct visits with participants several times a week for the first months and provide evidence-based treatment services to assist with stabilization and transition. Visits may decrease over time as a person becomes more stable in housing.Assist clients with interactions with landlord and neighbors, to encourage good neighbor/tenant practices and paying rent on time.Assess medical needs and ensure that clients are linked to appropriate primary care and specialty services. Follow up with participant medical care and pharmacy to guarantee they have needed medication.Provide ongoing evaluation of any presenting psychiatric or addiction symptoms and assess the need for supportive services. Link clients to internal and external resources, including mental health and addiction treatment services.Perform risk management duties to promote health and safety onsite. Coordinate with apartment maintenance staff when needed and assist participants in making work requests and filing grievances as needed.Respond appropriately to resident emergencies, conflicts, and behavioral concerns or conflicts. Use crisis prevention and intervention techniques to handle acute incidents of physical and mental health crisis and aggression.Engage participants in their homes and in the general community.Assist participants in renewing their voluntary commitments in the lease and program guidelines.OTHER RESPONSIBILITIES:Participate in staff training and development activities as directed.Participate in weekly staff meetings.Become certified in Homeless Management and Information Services database to enter services and view records, as needed.
