Job summary

Roof Above Inc. provides comprehensive homeless services across nine campuses in Charlotte, including Forest Point Place PSH.

The program case manager will work with a multidisciplinary team to help tenants secure housing, address basic needs, and access community resources. Responsibilities include home visits, crisis management, and coordinating with landlords and service providers to ensure stability and housing retention for adults who have experienced homelessness.

Qualifications

• Bachelor’s degree in a human services field or higher; master’s degree preferred.
• Minimum three years of case management experience with adults who experienced homelessness and mental illness or substance use.
• Experience with harm reduction and housing-first philosophy.

Responsibilities

• Provide case management to help participants transition from homelessness to housing.
• Manage caseload of ~18–19 individuals.
• Conduct home visits several times weekly to support stabilization and access to services.
• Assist with housing stability tasks: rent, referrals, and landlord coordination.
• Link clients to health, mental health, and substance use services; coordinate with medical/pharmacy care.
• Document progress and maintain client records in HMIS; monitor safety and crisis needs.
