Job summary

CarePlus NJ, Inc. is seeking a dedicated Case Manager to support the Housing Supportive Services program in Rochelle Park, NJ.

You will provide case management for NJ Family Care members at risk of homelessness, focusing on housing stabilization, intake assessments, and resource referrals. The role requires 1–2 years of experience with homeless or high-need populations, a Bachelors degree, and a valid driver's license.

Qualifications

• Bachelors degree in health, social work, psychology, or related field required.
• 1–2 years experience helping homeless or high-need populations preferred.
• Valid drivers license and insurability for primary driving duties.

Responsibilities

• Provide case management to NJ Family Care members at risk of homelessness.
• Assist with screenings, intake assessments, eligibility, and paperwork.
• Coordinate housing, tenancy maintenance, and sustainability plans.
• Document services in clinical records with time/date stamps.
• Link clients to community resources and crisis intervention as needed.
• Maintain client confidentiality and ethical boundaries.
