Job summary

Qualifications

• Bachelor's degree in social work or related field, or relevant case-management experience.
• Experience with social service agencies and grants.
• Strong verbal and written communication skills.
• Ability to manage a client caseload and work with diverse populations including people with disabilities.
• Valid driver’s license with clean driving record.

Responsibilities

• Oversee the implementation of individualized service plans for assigned clients.
• Coordinate services with community agencies to meet housing stability goals.
• Maintain accurate data entry and documentation in the agency database.
• Monitor and update Individual Action Plans (IAP) and report concerns to supervisors.
• Collaborate with interdisciplinary team members to address client needs and crisis situations.
