Job summary

Qualifications

• Bachelor's degree in Human Services, Social Work, Behavioral Science, or a related field with three years of directly related case management experience.
• Experience providing direct case management including needs assessment and action planning.
• Valid State Driver’s License required.

Responsibilities

• Conducts comprehensive assessments to identify homelessness causes, needs and strengths.
• Develops service plans and guides clients toward measurable steps to achieve housing stability.
• Coordinates with community resources and assists with applications for income support and disability benefits.
• Maintains HMIS data and prepares discharge plans and program involvement.
