Volunteers of America Colorado is seeking a dedicated Case Manager 2 to direct and facilitate delivery of support services for participant households. You will perform needs-based assessments, engage in community outreach, provide case management, and coordinate program-wide services with a focus on best practice standards.

• Bachelor’s degree in human services, social work, or a closely related field and 1 year of direct, full-time case management experience.
• OR Master’s degree in social work, counseling, psychology or a closely related field.
• Must possess a Colorado driver’s license and state-mandated automobile insurance; personal vehicle required for travel.
• Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.

Responsabilidades

• Provides case management services to Program Clients.
• Identifies and serves the unique needs of participant households using screening and assessment tools; ensures completion of needs-based screenings.
• Understands and implements assigned program per funder and VOA standards.
• Conducts community outreach to identify eligible participants with barriers to housing stability.
• Supports team as a resource for problem-solving and staying current with field practices.
• Attends orientations, trainings, staff meetings and related events as required.
• May provide training and mentorship to team members and stakeholders on best practices.
• Documents all client interactions with required eligibility and demographic data.
