Job summary

Volunteers of America Colorado (VOAC) is seeking a Full-Time Case Manager for the Rapid Rehousing Plus Care (RRH+C) and Shelter into Rapid Rehousing (SIRR) programs in the Denver area. You will perform needs-based assessments, coordinate housing navigation, and provide wrap-around services with community providers to help participants obtain and maintain permanent housing.

The role requires a Spanish-English fluency, a Colorado driver's license, and the ability to travel across the seven-county

Qualifications

• Bachelor’s degree in human services or related field or related experience.
• Colorado driver’s license and state-mandated automobile insurance.
• Fluency in Spanish (speaking, reading, and writing).

Responsibilities

• Coordinate wrap-around services for clients with the team and community service providers.
• Conduct needs-based assessments and housing navigation.
• Provide intensive case management for households with high barriers to housing stability.
• Transport participants as needed for case management.
• Attend intakes, housing orientations, lease signings and ongoing education opportunities.
• Maintain confidentiality and document appropriately in data systems.
