Job summary

Volunteers of America Colorado is seeking a full-time Case Manager for the Rapid Rehousing Plus Care (RRH+C) program in the Denver Metro area. You will deliver wrap-around housing services and intensive case management to participants experiencing homelessness, coordinating with partners to secure stable housing.

The role requires conducting needs-based assessments, assisting with housing navigation and landlord outreach, and maintaining grant-compliant documentation.

Qualifications

• Bachelor’s degree in human services, social work, or closely related field.
• Colorado driver’s license and state-mandated automobile insurance.
• Fluent in English; bilingual Spanish preferred.
• Experience in housing/homelessness services a plus.

Responsibilities

• Provide wrap-around case management for RRH+C participants.
• Conduct needs-based assessments and outreach in the Denver Metro area.
• Assist with housing navigation, landlord recruitment and mediation.
• Coordinate with service providers and multidisciplinary teams.
• Ensure timely reporting and data quality in grant systems.
• Transport participants for case management as needed.
