Volunteers of America Colorado seeks a full-time Case Manager for its Rapid Rehousing Plus Care (RRH+C) program in the Denver Metro Area. You will perform needs-based assessments, conduct outreach, and provide wrap-around housing services while coordinating with partner agencies.

A personal vehicle is required for program duties. The role requires fluency in Spanish and English, a Bachelor's degree in a related field, and a Colorado driver's license.

Qualifications

• This role requires applicants to be fluent in Spanish and English.
• Bachelor's degree in human services, social work, or a closely related field is required.
• Colorado driver's license and state-mandated automobile insurance are required.

Responsabilités

• Conduct needs-based assessments and provide wrap-around case management.
• Coordinate housing navigation and assist with securing housing.
• Support housing retention and connect clients to income resources and public benefits.
• Attend meetings, trainings, and collaborate with multi-disciplinary teams.
• Transport participants as needed and maintain confidential data records.
