Volunteers of America Colorado seeks a Case Manager for the Back Home SSVF program, a Rapid Rehousing initiative that uses a 90‑day model to stabilize housing for veterans experiencing or at risk of homelessness. You will primarily provide housing case management and connect veterans to benefits, VA services, health referrals, and financial health supports.

The role requires four years of direct human services experience, a valid Colorado driver’s license and insured vehicle.

Qualifications

• Four years of direct experience in the provision of human services/related experience or education.
• Must possess a Colorado driver’s license and state-mandated automobile insurance.
• Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available).
• Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.

Responsibilities

• Directs and facilitates the delivery of appropriate support services for participant households.
• Conducts needs-based and program eligibility assessments.
• Engages in community outreach and provides case management services.
• Coordinates program-wide services and applies field-relevant best practices.
