Job summary

Hope the Mission is seeking a part-time Housing-Focused Case Manager in Sun Valley, CA. The role emphasizes rapid placement into permanent housing using Housing First, low-barrier approaches, and trauma-informed care. You will perform assessments, develop action plans, and coordinate housing navigation and documentation.

Responsibilities include maintaining HMIS documentation, collaborating with housing partners, and participating in team meetings to support participant stability.

Qualifications

• Bachelor's degree in Social Work, Human Services or equivalent experience
• 1+ year of homeless services, housing navigation, or case management OR 3+ years with homelessness
• Knowledge of Housing First, Harm Reduction and trauma-informed care practices

Responsibilities

• Provide housing-first case management with emphasis on rapid permanent housing placement
• Assess barriers, preferences, and eligibility for housing resources and subsidies
• Develop and update Housing Action Plans with clear goals
• Maintain regular contact with participants for housing progress and motivation
• Coordinate move-in and housing navigation activities
• Document all services and ensure HMIS data is updated within 48 hours
• Collaborate with internal teams and external housing partners
• Participate in case conferences, supervision, and team meetings
