Job summary

Project We Hope is seeking a Case Manager in Palo Alto to manage a caseload of up to 25 clients, conducting assessments and developing individualized service plans focused on housing and wraparound supports.

You will provide linkage to housing, income, healthcare, behavioral health, and other resources, document sessions, and meet with guests weekly. Requires a Bachelor’s in social work or related field and at least 1 year of relevant experience.

Qualifications

• Experience in shelter or disenfranchised populations.
• Bachelor’s degree in social work or related field.
• Certification: Case Management/Care Coordination.
• Strong teamwork and communication skills.
• Knowledge of community resources and crisis response.

Responsibilities

• Provide holistic and comprehensive case management services: intake assessments, benefit assessment, goal setting, and long-term care plans.
• Maintain a caseload of up to 25 clients.
• Meet with clients bi-weekly; majority in-person when possible.
• Connect clients with community resources and document referrals.
• Input data into Salesforce and HMIS within 48 hours.
• Maintain confidentiality and professional conduct.
