Responsibilities

• Provide housing-focused case management using Housing First and trauma-informed approaches with emphasis on rapid permanent housing.
• Conduct participant assessments to identify housing barriers and eligibility for resources.
• Develop, implement, and update Housing Action Plans with measurable goals.
• Maintain regular contact with participants focusing on progress, problem-solving, and motivation.
• Support tenants in understanding rights, leases, and tenancy expectations.
• Assist with housing navigation activities including applications, referrals, and move-in logistics.
• Coordinate with housing partners to ensure access to services and resources.
• Document all services and contacts in HMIS or approved systems within required timelines.
