Job summary

Hope The Mission in Los Angeles seeks a Housing-Focused Case Manager (PT & FT) to guide participants from interim housing to permanent housing using Housing First and trauma-informed approaches. You will work under the Program Manager, conducting assessments, developing action plans, and coordinating move-in logistics.

Responsibilities include ongoing participant engagement, HMIS documentation, and collaboration with housing partners to shorten shelter stays and build long-term stability.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or equivalent experience.
• Minimum 1 year in homeless services, housing navigation, or case management OR 3 years in homelessness-related work.
• Knowledge of Housing First, Harm Reduction, Trauma-Informed Care, and motivational interviewing.

Responsibilities

• Provide individualized housing-focused case management using Housing First and trauma-informed approaches.
• Conduct participant assessments to identify housing barriers, preferences, and eligibility for resources and subsidies.
• Develop, implement, and regularly update Housing Action Plans with clear goals for permanent housing.
• Maintain regular contact with participants focused on housing progress and motivation.
• Assist with housing navigation activities including applications, document collection, and move-in logistics.
• Document all services and housing activities accurately in HMIS or approved data systems.
