Job summary

The People Concern is seeking a ICMS Bilingual Case Manager in Sun Valley, CA to support clients facing homelessness. You will manage a caseload and coordinate services to secure and retain housing, with emphasis on harm reduction and trauma-informed care.

You will collaborate with DMH/DHS, assist with benefits applications, and transport clients as needed within the greater Los Angeles area. The role is 100% onsite, Mon-Fri, 8:00am-4:30pm.

Qualifications

• Bilingual Spanish language proficiency required to engage with clients.
• High School Diploma or GED or equivalent.
• Willingness to transport clients in personal vehicle.
• Able to work in harm reduction and trauma-informed settings.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity clients for intensive case management.
• Collaborate with LA County DMH and DHS programs to coordinate care.
• Screen, enroll, and orient clients to program policies and goals.
• Complete biopsychosocial assessments and help plan individualized care.
• Coordinate appointments, transportation, and follow-up services to access health care.
• Maintain confidential client records and accurate progress notes.
• Work with property managers and partners, participating in weekly staff meetings.
