Job summary

Hope the Mission in North Hollywood, CA is seeking a Housing-Focused Case Manager to provide housing-first, trauma-informed case management to participants in interim housing, with emphasis on rapid permanent housing placement.

Under the supervision of the Program Manager, you will assess barriers, develop action plans, coordinate housing navigation, document in HMIS, and collaborate with partners to help participants achieve stability and housing.

Qualifications

• Bachelor's degree in Social Work, Human Services, Psychology, or equivalent experience.
• Knowledge of Housing First, Harm Reduction, Trauma-Informed Care, and motivational interviewing practices
• HUD guidelines, subsidized housing programs, and Coordinated Entry System preferred
• Experience with HMIS or similar data systems is preferred.

Responsibilities

• Provide housing-first, trauma-informed case management to place participants into permanent housing.
• Assess housing barriers, preferences, and eligibility for resources.
• Develop and update Housing Action Plans with clear goals.
• Maintain regular contact to monitor progress and motivate participants.
• Assist with housing navigation: applications, documents, interviews, viewings.
• Coordinate with housing partners and agencies for services.
• Document all services in HMIS or approved systems on time.
• Track outcomes: referrals, placements, length of stay, exits.
• Ensure compliance with policies, safety protocols and reporting.
• Collaborate with multi-disciplinary teams and participate in case conferences.
• Coordinate with site staff, operations, and leadership on housing-related concerns.
• Conduct field-based work; off-site housing appointments and meetings.
• Support populations and flexible scheduling as needed.
