Job summary

City Net is seeking a Housing-focused Case Manager in Anaheim to provide outreach-based support for individuals and families experiencing homelessness. You will assess needs, connect clients to housing resources, and document data for HMIS/CES systems while collaborating with community partners.

The role requires strong communication, data accuracy, and the ability to work independently in a dynamic environment, with potential early mornings, evenings, and weekends.

Qualifications

• Must be eligible to drive a company vehicle and meet insurance requirements.
• Experience cultivating productive relationships with stakeholders.
• Understanding importance of quality and timely data collection and reporting.
• Excellent professional boundaries and communications.

Responsibilities

• Conduct client needs assessments and help prioritize housing and services.
• Support housing placement by assisting with programs and applications.
• Advocate with partner agencies and accompany clients to appointments when needed.
• Collect and document data accurately for HMIS and CES systems.
• Meet productivity and outcome goals and comply with funder requirements.
