Job summary

City Net is seeking a full-time Case Manager in California to provide housing-focused case management and supportive services to individuals and families experiencing homelessness, primarily through street outreach. You will meet clients where they are and help them access housing programs and coordinated care to achieve stable housing.

Responsibilities include assessments, housing placement, referrals, advocacy, and data entry into HMIS/CES, with emphasis on meeting productivity and outcome

• Must possess strong verbal and written communication skills.
• Ability to work with diverse populations and stakeholders.
• Excellent professionalism and boundary setting.
• Experience coordinating with community resources and service providers.

Responsibilities

• Conduct client needs assessments to identify housing and support requirements.
• Assist clients in accessing housing programs and completing applications.
• Develop and monitor long-term housing plans with clients.
• Refer clients to medical, mental health, and other supportive services.
