Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I to support residents experiencing homelessness by developing and implementing care plans, coordinating services, and advocating for ongoing support. You will deliver outreach, coordinate with health teams, and ensure accurate documentation in the EHR while maintaining client confidentiality and trauma-informed approaches.

• High School Diploma or GED required; Bachelor's degree preferred.
• 1–2 years experience with diverse populations experiencing mental health challenges or homelessness preferred.
• Bilingual or multilingual skills preferred.

Responsibilities

• Manage a caseload of clients with high-service utilization and disabilities.
• Coordinate services and referrals with community partners.
• Maintain client records and data in the EHR.
• Provide crisis intervention and support as needed.
